There is a new Online form for reporting Accidents and Incidents you will find it on the OH&S web site.
There are also links to it from the Intranet pages Staff Development - Useful Links &
Library Links. I have also linked it under the A-Z index under Accident & Incident and OH&S Accident & Incident.
Anybody with a UTAS Pop account can complete this form.
The online form goes directly to Rodney Teague - he then advises the library.
Printed forms now go to Rodney Teague Private Bag 102
